A Case of Iatrogenic Complex Regional Pain Syndrome

Abstract
The author observed a case of iatrogenic complex regional pain syndrome in a 43-year-old man after an in-car accident without nerve injury. He was suffered from an unexplained pain over nine months. He received analgesics, physical therapy and various kinds of pain blocks without any symptomatic improvements. New diagnostic terms were added including the reflex sympathetic dystrophy, the cervical disc herniation and spondylosis. He received abundant pain blocks including transforaminal anterior epidural block, radiofrequency thermocoagulation, epidural adhesiolysis, caudal block, discography, epidurography, intradiscal steroid, dorsal root medial branch block, prolotherapy, and intramuscular stimulation. He is still in severe pain despite of abundant pain killing procedures over 9 months for relatively mild superficial injuries. It is more appropriate to describe a patient as having "regional pain of unknown origin" than to diagnose CRPS. By the incorrect diagnosis, the patient became a patient with an iatrogenic CRPS. The author report such a case, including clinical presentation, management, and review some medicolegal issues.
